THE FOLLOWING FORMS ARE USED IN COOK COUNTY FOR DIVORCE AND PATERNITY CASES.

Divorce:

- Disclosure Statement:

A Disclosure Statement must be exchanged between the parties in all cases where the parties have not entered into an agreement disposing of the issues except for those cases where the other party is in default for failure to file an Appearance or plead. A Disclosure Statement must be exchanged between the parties prior to any hearing on maintenance or child support.

- Affidavit as to Military Service:

If a person is served by personal service or publication and has not filed an Appearance in the cause of action, the Petitioner must file an Affidavit as to Military Service prior to the prove-up (uncontested trial).

Paternity:

A Disclosure Statement must be exchanged between the parties prior to any hearing on child support.
